Please indicate the address where your cheque should be forwarded: For example:

  1. In Canada: address where it should be mailed.
  2. Bank in Panama: indicate the name of the Bank.
To be picked up by an authorized person: indicate full name of the person and Cedula number. The authorized person must obtain a card from the Social Security Department entitled “Autorización de Entrega de Cheque”. The card should be completed and signed by you and sent together with a Proof of Life to the Consulate of Panama.
If you can’t go to the Consulate in person, you have to go before a Notary Public to get the Proof of Life duly notarized, and then mail it to the Consulate.
After being certified by the Consulate, you can send the document directly to the Social Security Department (Tel: 011-507-261-4413) by courier to this address:

NOTE: THE DOCUMENT OF PROOF OF LIFE SHOULD BE IN SPANISH AND HAS TO BE RENEWED EVERY 6 MONTHS.
