Requirements:

1. Birth Certificate
2. Police Record
3. AIDS test issued by a Recognized Canadian health Center (Canadian Ministry of Health)
4. Certificate of single status.
5. Certificate of Baptism (If you are going to get married in a catholic church).
6. Copy of your Canadian Passport, previously certified by a Canadian notary.

Note:
The documents mentioned need to be authenticated by the Department of Foreign Affairs – Authentication and Service of Documents (125 Sussex Drive, Ottawa)
There is a fee of U.S. $30.00 for each document and it has to be a Money Order addressed to the Embassy of Panama – Consular Section. (No personal cheques will be accepted)
